I am authoring a paper for my scholarly initiative to adopt the Chronic Care Model for patients with HIV/AIDS. In my research to date, I have discovered that this patient population requires significant support and guidance throughout the life span to effectively manage a serious chronic disease, and I have recognized that patients benefit from continuous monitoring and evaluation to improve quality of life and to improve their daily management of this complex disease. From a nursing perspective, working with HIV/AIDS patients in this capacity demonstrates the importance of the Chronic Care Model in advancing plans of care and in minimizing the long-term risks of the disease and its impact on human health. The Chronic Care Model exemplifies the value of communication and collaborative care in HIV/AIDS management and to determine what steps are necessary to sustain a positive and meaningful strategy to improve quality of life and minimize symptoms for this patient population.

I am requesting an IRB exemption because this scholarly initiative is based upon prior research evidence and will not involve new human subjects or incorporate an invasive method from its inception to implementation. The scholarly initiative is designed to address some of the issues related to the Chronic Care Model that have been identified in in prior studies, does not involve the inclusion of new human subjects, and aims to improve upon existing principles that have already been identified in this subject area. These principles will be applied in order to understand the needs of the HIV/AIDS population and to minimize changes in viral load and symptom risk. Therefore, this study will not require an IRB protocol and subsequent approval from the IRB committee.
