Assignment specifics – Form layout
Create a form that can be used as a checklist, based on items 1-12 found in section 1.5 of the attached document to verify whether a business is or is not PCI compliant. The form should contain the following items:
Item
A listing of all (12) PCI compliance audit points
A brief ((1) sentence/phrase) explanation of all (12) PCI compliance audit points <–This detail can be listed under each item or placed on a different page/set of pages
Form checklist items – Provide a location for
Marking whether each item is PCI compliant, non-Compliant, Not Applicable or Not tested
A location to leave a brief explanation of why each non-compliant item is non-compliant
Form sign-off items. Please provide locations for indicating:
The date of the audit
Name of organization being audited
Address of the organization
Contact information of the organization – Person to contact, their email address and phone number
Name and signature of the auditor along with their email address and phone number
In order to obtain credit
Upload an MS Excel spreadsheet, MS Word or .PDF document
