The research question for this DNP project is as follows: How can healthcare providers adapt the Chronic Care Model (CCM) to care for long-term survivors of HIV/AIDS? This research question requires the discovery and analysis of new and existing evidence in order to be effective in achieving support for the CCM for HIV/AIDS patients who possess longer survival rates. It has been demonstrated that this model is essential in supporting new directives for this patient population to improve their quality of life over the long term (Chu & Selwyn, 2011). Therefore, prior evidence must be identified that may be adapted to chronic care for HIV/AIDS patients with the intent to promote improvements in self-management for the disease (Schulman-Green et.al, 2012). At the doctoral level, nurses must uncover new concepts and apply existing theories to view HIV/AIDS patients with a new perspective that will support information analysis and decision support in this capacity (Pasricha, Deinstadt,  Moher, Killoran, Rourke, & Kendal 2013; Robbins et.al, 2012). This strategy will enable patients to receive dedicated care, treatment, and other services that will positively impact their health and quality of life over the long term.

In order to carry out the goals of this project, the intent is to identify areas in the workplace environment where I am able to apply concepts related to HIV/AIDS patients that have been successful for patients with diabetes and CHF. Since my experience involves psychiatric care, I believe that my knowledge and experience will support patients with HIV/AIDS effectively and encourage improvements in chronic care and utilize existing knowledge to improve outcomes. I will work in conjunction with my preceptor on this project, as she has significant experience in the DNP role and works extensively with HIV patients. I will gain valuable exposure to pregnant women with HIV, hematology/oncology patients with HIV, and patients who visit the STD clinic. These experiences will provide significant benefits as I move forward with my DNP project and establish a basis for growth within this area.
